FAST TRACK OPTION

With standard applications it can take up to 28 days from the receipt of all required documentation for your card to arrive. For an additional payment of £60.00 (plus standard card fee) you can request your application to be fast tracked. When an application is fast tracked, we aim for you to receive your card within 4 working days. Fast-track applications require a correctly completed application form, provision of all required supporting documentation and cleared payment. Applications that are submitted without the correct supporting documentation and/or full payment will be returned. If you are sure you can meet these criteria, please select the fast- track option above to fast-track your application.


SECTION 3: Health, Safety and Environment Training


1. CITB Health, Safety and Environment (HS&E) Test

If you have passed the CITB HS&E test within the past 2 years please tick this box and ensure you send a copy of your test pass letter with this application.


2. Other applicable Health and Safety Tests

If you have passed any other acceptable Health and Safety tests/courses within the last 2 years please ensure you send a copy of your test/course certificate with this application. For a list of acceptable tests, please visit our website:www.jib-pmes.org


Please Note: Those providing a NARIC equivalence report are NOT entitled to any NVQ/SVQ exemption


Failure to provide a copy of your H&S test pass could result in your application form being returned.


Also make sure you have completed an acceptable UK qualification in ‘Health and Safety in a Construction Environment’ or undertaken an equivalent course. A full list of acceptable alternative courses/qualifications is in the guidance notes in Section 6 of this application form and also on the JIB-PMES website. www.jib-pmes.org


SECTION 4: Labourer/Plumber's Mate Green card


Labourer/Plumber's Mate Green Card

Applicants for this type of card MUST choose one of the following designations to appear on the front of the card:


If these boxes are left unticked, the occupation ‘Labourer’ will appear on your CSCS card by default.

Since March 2017 it has been a requirement for anyone applying for a Green Plumber’s Mate or Labourer card to complete either the:

  • - Level 1 Award in Health and Safety in a Construction Environment
    • Or
  • - An appropriate equivalent Health & Safety course/qualification.

In addition to passing the CITB HS&E test.


All card applicants must therefore tick to confirm they meet the requirements outlined at either 4A, 4B or 4C as detailed below.


4A. L1 Award in Health and Safety in a Construction Environment

If you have achieved the L1 Award in Health and Safety in a Construction Environment, please tick this box and upload a copy of your certificate.


4B. Appropriate equivalent Health & Safety course/qualification

Appropriate equivalent qualifications are listed in the Guidance Notes section of the pdf application form and on the JIB-PMES website: www.jib-pmes.org. If you have achieved one of these, please tick this box and upload a copy of the certificate you received.


4C. Plumbing ‘Technical Certificate’ course/qualification

Applicants who have completed the City and Guilds 6129 or 6035 Plumbing Studies qualifications (or BPEC/EAL equivalent qualifications) will not have to complete the L1 Award in Health and Safety in a Construction Environment qualification. These qualifications are also classified as appropriate equivalent qualifications for the purposes of Green Plumber’s Mate/Labourer card applications. If you have achieved one of these qualifications, please tick this box and upload a copy of your certificate.



Employment details

Please provide details of your current employer or a company you have worked for (not a domestic client). If you are self-employed, or currently unemployed, we will accept a company you have sub-contracted for or a client you have completed work for. It is not acceptable to enter your own company details.



* Please note that checks will be made with the contact person to ensure that details included in this application are accurate and that you are, or have been employed in the PHMES sector.

By signing the below declaration, I certify that the details in this application are correct and complete to the best of my knowledge.

I understand and agree that the information in this application will be used by the JIB-PMES CSCS / UK-PHMES and its suppliers for the purpose of processing my JIB UK-PHMES card application. As part of the application process it may also be necessary to share your information with employers, labour agencies, training providers and awarding organisations for verification purposes.

I understand that my personal data will be stored in a secure database and processed in accordance with the requirements of the JIB-PMES Privacy Statement (which can be accessed on the JIB-PMES website: www.jib-pmes.org); the JIB-PMES Privacy Policy and the Data Protection Act 2018.

I also agree that the information contained in this application may be used by the JIB-PMES or shared with selected third parties in relation to the provision of other services associated with my employment in the plumbing and mechanical engineering services industry.


Important Information:

  • * I acknowledge that full payment will be taken at the time of application and is non-refundable. I understand that I have up to 90 days from submitting my application form to provide any missing or additional information required for my application. If this process takes longer than 90 days, I understand that I am liable for the payment of another application fee.
  • * The card remains the property of the JIB-PMES and can be withdrawn at any time.
  • * If all the relevant documentation is not provided with the application, it will not be processed.
